11/5/2023 EnchantaVaughn Approved Behavior: Client was happy to see her counselor and thanked her for coming over.. Client and QMHP discussed clientâs behavior at home. Client followed all directions given by mom and counselor. Client made constant eye contact during session and displayed a calm demeanor without getting angry or frustrated with durataion lasting 2-3 times of tempter taturms. Behavior: Mother shared client struggles with making positive decisions with her behaviors lasting daily for 2-3 minutes in duration. Intervention: QMHP monitored client individualized behavior modifications in collaboration with her mom and other relatives in the household. QMHP motivated client to respond to exercise and express her feelings. QMHP offered active listening skills. QMHP provided client with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP encouraged client to communicate to her mother when she needs help. QMHP praised client for staying on task. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the important issues. Intervention: QMHP supported client displaying positive social interaction with peers and authority. QMHP inquired about at home activities. QMHP applauded client for being a leader and developing into a mature individual. QMHP reiterated to client the benefits of following the rules and of the privileges she can receive when she does so. QMHP assisted client with identifying privileges that she would like but does not receive due to her negative behaviors; in efforts to stimulate her thinking about cause and effect of her behavior choices. Response: Client utilized the resources provided and seem to be doing better with managing symptoms of her disorder. Client was ready to work and was more driven to try and stay positive for the remainder of the day. Client displays good participation in educational setting. Client displays appropriate behaviors at this time. Client understands that breaks are a privilege and she have to earn it. Client demonstrates problem solving by staying calm and using her senses. Client acknowledges expectations and reviews goals and objectives with counselor. Response: Client utilized coping skills and impulses started decreasing. Client was observed strategizing a plan to better herself by writing down her thoughts and feelings and exploring her creative side with arts and crafts. Client was taught to deep breathe until she is ready to process her feelings. Client was reported displaying age appropriate behaviors and was productive and studious throughout the session. Client thanked QMHP for the support and assistance and helping her stay on track with her goals and objectives. Client was able to be redirected successfully. Client stated she would try better self-management as the day pursues.
